To file a complaint about a specific food establishment, you must first identify it by name or location.  Search for an establishment by license type, or, if you know it, the establishment's license number.  Please enter only the first few letters of the name of the business you are looking for and select the city in which it is located and you will get the best results.  Once you have entered the information, click the "search" button to generate a results page.

The results page displays a list of all licensed establishments meeting your search criteria.  Once you have clicked on an establishment name, a form will appear which will allow you to enter the details of your complaint.
